Access to Information and Privacy
As stated in the Letters Patent of its incorporation, the Federal Public Service Health Care Plan Administration Authority is subject to the Access to Information Act and the Privacy Act, as amended from time to time.
If you would like to make a request under the Access to Information Act, you must submit the following:
- A completed Access to Information Request Form
- A $5.00 application fee by cheque or money order made payable to the Federal PSHCP Administration Authority
- Note: the Administration Authority does not have an account with the Receiver General for Canada
